The community you choose
Contracted communities pay us a referral fee, a percentage of the resident’s first month’s fees, only when a placement is made.
There’s no pressure to choose any particular community. Colorado requires referral agencies to disclose how they’re paid, so here it is, plainly.

We share details on the communities that best fit your needs.

We arrange tours at communities you choose and join you, in person or virtually.

We collaborate with the chosen community to organize the move and ensure that the paperwork is in place. We are available to point you to resources for downsizing and moving.

Who you’ll work with
Our expertise
We are Certified Senior Advisors® who have specialized knowledge about seniors’ physical, developmental, social, and practical needs and have agreed to a professional code of ethics.
Transitions Senior Care is an unbiased advocate for clients with access to all communities throughout the Denver Metro area and beyond.
Transitions Senior Care functions as an Independent Referral Agent for the community and is paid a percentage of the client’s first month fees by the community.
Both advisors hold the Certified Senior Advisor® credential.

My mother had a stroke and we needed to transition her to assisted living from her apartment on short notice. I was not looking forward to calling different places and setting up appointments for tours. We act as independent contractors for communities across the metro area. Contracted communities pay us a fee when you move in. There is no pressure to choose any particular community.
What happens on the first call?
We start with a needs assessment. We ask about health and medical history, finances, where you’d like to live, the amenities that matter and your timeline. That tells us which communities are worth a visit.
How many communities will you suggest?
Every client is different. We send you information on matched communities based on our recommendations, then set up tours and join you on those of interest, in person or virtually.
Do you help after we choose a community?
Yes. We collaborate with the community to organize the move and ensure that the paperwork is in place. We are available to point you to resources for downsizing and moving.
How do families pay for assisted living or memory care?
Most families pay privately, from savings, retirement income or long-term care insurance. Medicare doesn’t pay for long-term custodial care (Medicare). Eligible veterans and surviving spouses on a VA pension may qualify for an added monthly payment called Aid and Attendance (VA). Some communities accept Health First Colorado (Medicaid). We’ll talk through what applies to your family and point you to the right professional for financial or legal questions.
