  2. Get started with a phone conversation or meeting to discuss medical history, health concerns, financial needs, geographic preferences, important amenities and timeline.
  3. Transitions Senior Care (TSC) arranges tours and joins you and your family in exploring communities until your decision has been made.
  4. We provide resources to assist you with downsizing, moving, and more–anything you need to coordinate your move.
  5. We follow-up with you the day after move-in, a week after move-in, and 3 months after move-in.

Our Expertise

  • We are Certified Senior Advisors® who have specialized knowledge about seniors’ physical, developmental, social, and practical needs and have agreed to a professional code of ethics.
  • TSC is an unbiased advocate for clients with access to all communities throughout the Denver Metro area and beyond.
  • TSC functions as an Independent Referral Agent for the community and is paid a percentage of the client’s first month fees by the community.
